Immunization Requirements for C.C.S.D. Students

Statutory Requirements and C.C.S.D. Procedures

K - 5th Grade Students (includes all transfer students to CCSD):

5 DTP/TD minimum with at least one does administered on or after the 4th birthday.

4 Polio minimum

2 MMR minimum

3 Hepatitis B

6th Grade Students:

4 DTP/TD minimum

4 Polio nimimum

2 MMR minimum

7th - 12th Grade Students:

4 DTP/TD minimum + 1 additional TD

4 Polio minimum

2 MMR minimum

3 Hepatitis B

If a 4th DTP was administered after the 4th birthday, a 5th dose is not required. Administration of 4 doses is considered optimal.

If a 3rd Polio dose was administered after the 4th birthday, a 4th dose is not required. Sequential OPV or IPV may be used but the final dose must be given after the age of 4, even if this means a 5th dose.

With the 1st MMR dose on or after the 1st birthday and a 2nd dose should be administered at least one month af after the first dose.

An adolescent Tetanus Diphtheria (TD) booster is required for all 7th, 8th and 9th grade students who have not received a tetanus or diphtheria vaccination during the previous five years. subsequently, TD boosters are recommended every 10 years.

For students 7 years of age or older who were not immunized prior to the 7th birthday, a total of 3 doses of TD vaccine, 3 doses of polio vaccine and 2 doses of MMR vaccine are required.
