You must have a physical examination, by a doctor, before participating in any activity (practice included). You can obtain a form at the Sage Valley front office or at your doctor's office. For the 2011-2012 school year any physical dated June 1st or later will work for all activities during the year.

Below are the dates of the low cost ($25.00) physicals offered at the WORI buliding (upper level) on Stocktrail Ave. You can register online at www.ccmh.net or www.cchcf.com. You can also call beginning on June 1st to schedule an appointment. The number is 688-6666. Please wear shorts, t-shirt, and slip-on shoes to the appointment. Times are from 6-8 p.m.
